Индивидуальный подход к выбору оптимальной терапевтической тактики в современных условиях позволяет значительно улучшить результаты лечения больных с HER-2-позитивным раком молочной железы, в том числе и с распространенными стадиями.
В арсенале онкологов существуют 2 препарата направленного действия против HER-2-рецепторов: трастузумаб и лапатиниб. Лапатиниб, являясь малой молекулой, проникает через гематоэнцефалический барьер и обеспечивает высокий терапевтический эффект при метастазах рака молочной железы в ЦНС. В данной статье представлен клинический случай применения лапатиниба у пациентки 32 лет с метастазами HER-2-позитивного рака молочной железы в головной мозг. Комбинация лапатиниба с капецитабином позволила продлить пациентке период жизни без прогрессирования заболевания на 22+ мес. Несмотря на локализацию метастатического очага в ЦНС, постоянный прием противоопухолевой терапии, пациентка вернулась к своей работе и полностью социально адаптирована.

Individual treatment strategy can significantly improve treatment outcomes in patients with HER2+ breast cancer (BC), including advanced stages. There are two targeted agents, actually approved for the treatment of HER2+ BC: trastusumab and lapatinib. Lapatinib is a small molecule which was shown to be able to cross the blood-brain barrier. This makes lapatinib efficient in treatment of patients with brain metastases.. In this report we present a case of the 32-year-old female with HER2+ BC and brain metastases, who received a combination of lapatinib and capecitabine. This combination resulted in the increase of time to progression for up to 22+ months. It also significantly improved the QoL and social adaptation of the patient and enabled her to work.
